Formally launched in July 2019, the WE CARE HOME HEALTH is an outreach arm of the We Care Clinic to augment the healthcare needs of established patients and assure continued care in the home setting. The team is composed of doctors, nurses, administrators, clergy and other members of the We Care Home Health Team.

For instance, Home health nurses provide one-on-one care for patients in their homes in terms of assisting patients with basic needs such as bathing and dressing, as well as more specialized services such as wound care, medication management, and IV therapy prescribed as medically necessary by the We Care Clinic Physician.

Enrollment Requirements:
1. Established care with and seen by a We Care Clinic Physician
2. A person with disability and
a. With extreme limitation of mobility
b. With stroke, wound care needs, reconciliation of medications and any deemed by the Physician as medically appropriate
3. PhilHealth insurance for hospital referral purposes only
4. Free for Cabeneans in Caba. If service is requested outside Caba, nominal transportation fee is shouldered by requesting party
